Curated archive record from NASA's 2023 historical software and automation incident catalog. NASA records an uncontrolled ISS attitude spin caused by erroneous thruster-firing software and classifies it as a close call. The catalog cautions that an incident's behavior may not mean software itself was the sole root cause. This is a public-source summary, not an independent eyewitness report.
