Curated archive record from NASA's 2023 historical software and automation incident catalog. NASA records that missing input data led flight computers to calculate an uncommanded pitch-up; the crew ejected successfully. The catalog cautions that an incident's behavior may not mean software itself was the sole root cause. This is a public-source summary, not an independent eyewitness report.
