Curated archive record from NASA's 2023 historical software and automation incident catalog. NASA says all four shuttle computers failed or disagreed during fueling, so fueling was stopped. The catalog cautions that an incident's behavior may not mean software itself was the sole root cause. This is a public-source summary, not an independent eyewitness report.
